Georgia Independent Care Waiver Program (ICW)

Program Overview

The Georgia Independent Care Waiver Program (ICW) is administered by the Georgia Department of Community Health (DCH) that comes under the Home- and Community-Based Waiver.  The ICW program assists adults between the age of 21 and 64 who are physically or intellectually disable by providing home nursing services. Individuals who are suffering from TBI or Traumatic Brain Injuries are also eligible to apply for the Independent Care Waiver Program. The services provided by the ICW program include personal and health support, support with choosing the right medication, medical equipment, emergency response services, and many more.

The purpose of the Waiver program is to assist adults and elderly citizens with physical or intellectual disabilities by making them part of a community. The services are provided either at home or at community care or daycare. Eligible individuals are provided community-based services to make them live their lives more independently and with their loved ones.

Program Objectives

The main objective of the Georgia Independent Care Waiver Program (ICW) is to help an adult in strengthening their self-maintenance while living in the same community. The program analyzes the situation of these physically or mentally disabled adults and comes up with different remedies to help ease their pain and get them out of challenging experiences they are suffering in their lives. The program helps older people with physical disabilities to attain self-sufficiency by providing an extensive range of community-based healthcare services.

Program Services

The Georgia Independent Care Waiver Program (ICW) provides a huge range of services. Some of which are,

  • Emergency Response Services provides 24 hours of telephonic services to people who need help on an urgent basis due to an emergency.
  • Out of Home Respite Care services provide relief to the caregiver temporarily by providing at-home or daycare services  to adults
  • Providing health and nutritional meals through the Home Delivered Meals Services
  • Personal care including shopping, respite services, meal preparing, and home maintenance service through the Personal Support Services
  • Adult Day Health services that include supervision of the adult at day time

Eligibility Requirements

The Independent Care Waiver is eligible for people, who fulfill the following eligibility requirements,

  • The applicant is a resident of the state of Georgia
  • The applicant is a permanent citizen of the U.S. or is  a legal alien
  • The individual is between the age of 21-64 and has a disability of TBI.
  • The individual is at the risk of being shifted to a hospital because the nursing home is not available however he/she is capable of staying home

How to Apply?

There are many ways to apply for the Georgia Independent Care Waiver Program (ICW) including,

  • Apply online

You can apply for the Georgia Independent Care Waiver Program (ICW) by going to the Department of Community Health’s (DCH) Division of Medicaid official website where you can find the online application form for the program. Here you will also find the application forms for other related programs.

  • Apply through the local district office

Applicants who are interested in the ICW program can visit the Department of Community Health’s (DCH)  office in your locality to make a personal application. The applicant will be provided with an application form that they will have to complete and submit to apply for the program benefits. After finding a nearby office, you can visit it to get guidance regarding the program eligibility and application procedures. You can visit the DCH office to find a detailed list of contact numbers and find the one for your area.

Once the form is filled and you have attached all the required documents, you can then submit the application form which will be further processed by the office. The selected applicants will be notified through phone or email and will get notified about to the next level.
